GSM 產品:

1. 販賣用電信終端設備型式認證申請書 (one model for one application)

2. 交通部電信管制射頻器材經營許可執照

3. 營利事業登記證

4. 代繳規費及領證委託書

5. 符合聲明宣告書 (二頻or 三頻)

6. IMEI 唯一保證書

7. 使用手冊: 需標明廠牌、型號

(1)應在中文使用手冊適當(顯著)地點標示(非手持之產品免)：

「減少電磁波影響，請妥適使用」

「SAR 實測值為：(請寫上實測值) W/Kg」

(2)中文使用手冊不能有簡體字

中文使用手冊來不及提出者請附英文使用手冊及後附之保證書：「提供中文使用說明書保證書」註: 當產品內含低功率射頻模組時，需加入以下低功率警語。

8. 使用規格資料: 規格內容須有廠牌、型號、使用電源、發射頻率、頻道數、發射功率 (與測試值不超過2dBm)、調變方式、Antenna Type and Gain

9. 電路圖或方塊圖: 需標明廠牌、型號

10. 天線規格: 包含所有module

11. SAR Report & Safety report

Note:

1. 以上文件1、2、3、4、8 及中文使用說明書保證書需加蓋公司大、小章

2. 請先確認廠牌，當所使用之廠牌非申請者所屬，也非製造者所屬，則需提所屬廠牌

公司之授權信。

3. 製造者為台灣廠商，則需附製造者之交通部電信管制射頻器材經營許可執照 (需登記有工廠廠址) 。

4. 如果申請者為外國製造商，則僅需提供該國工廠證明文件即可，無需提供交通部電信管制射頻器材經營許可執照; 但商品於進關時仍需提出台灣Agency 交通部電信管制射頻器材經營許可執照辦理。
